Autoimmunity occurs when the immune system attacks healthy tissue. These diseases are quickly becoming a major global health problem. The top 10 that are most common in the United States are:
  1. Hashimoto’s Disease (Hypothyroidism)
  2. Graves’ Disease (Hyperthyroidism)
  3. Rheumatoid Arthritis (Joints)
  4. Lupus (Multiple tissues)
  5. Sjogren’s Syndrome (Dry Eyes, Skin, and Mouth)
  6. Celiac Disease (Gut)
  7. Psoriasis (Skin and Joints)
  8. Multiple Sclerosis (Brain Cells)

Conventional diagnosis of autoimmune diseases is commonly done through a blood test that looks for antibodies against healthy cells. After these antibodies are found, conventional medicine recommends taking immune suppressive drugs and/or hormone replacement therapy for life – with risks that greatly outweigh the benefits. Functional medicine (advanced natural medicine) has powerful and natural tools that dramatically improve the lives of those with autoimmune diseases without these risks.

The reason why conventional medicine has failed with autoimmunity is because their model is not designed to look for the individual cause (or promoters) of the immune dysfunction. Two different people with Hashimoto’s hypothyroidism will have very different immune dysfunctions therefore will need different protocols. The key is to get a clear picture of the individual’s unique immune system can only be achieve through functional laboratory testing.

The Three Secrete Blood Tests

TH1 / TH2 Cytokine Test– Your white blood cells communicates with each other through proteins called cytokines. You immune system consists of mainly two systems: The first attackers (Th1 cells) and antibodies (Th2 cells). In a properly functioning immune system, the Th1 and Th2 cells should be in balance like a see-saw. When one of these systems becomes more powerful than the other, and autoimmunity can be triggered. The goal is to balance this system. Since many therapies used in natural medicine can influence this see-saw, it is critical to know which side is dominant in order to promote balance.

CD4 CD8 Ratio Panel (Immune System Analysis)– We have cells in our immune system called “helper cells” that help our white blood cells attack foreign innovators.  Other cells help turn off the immune attack (“suppressor cells”) when the fight is over. This important test it allows you and your doctor to know if a foreign innovator (bacteria, virus, food, environmental toxin, and/or heavy metals) are influencing the autoimmune attack or if the immune system is dysfunctional due to other causes like chronic stress or nutritional insufficiencies.

Vitamin D Test– Vitamin D is not actually a vitamin, it’s a hormone. A hormone that plays an extremely important role in the modulation, or balancing, of the immune system. Not only do most people suffering with autoimmune diseases have low levels of vitamin D but they also have genetic changes that keeps vitamin D from being used in the body. This means they many need to take very high doses of vitamin D in order to function properly. Since vitamin D influenced blood calcium levels it is necessary to have your vitamin D levels rechecked often until optimal levels are achieved. (The vitamin D test is also included in our Health Check Plus with Vitamin D blood test)

  • Eating less is not the answer to weight loss. Eating less calories can slow overall metabolism and potentially shift the calories you do eat into fat. Eating more will speed your metabolism as long as it comes from good quality foods. If a power plant is not burning its coal then even if the train only brings one car of coal, it’s going to pile up (weight gain).
  • The goal is to maintain a healthy lifestyle not to “go on a diet”. A diet may be used to aggressively break down barriers that keep you from losing weight but what most people want is to live a healthy life, not bounce from diet to diet. A diet is only a stepping stone.
  • The key is baby steps. Whenever there is a time gap between the present moment and the future, anxiety and frustration occur. The brain will try to adapt by thinking, “Let hurry up and get to our final goal because I don’t want to be anxious or frustrated anymore.” This is when you go from not working out or eating right to hitting the gym 6 times a week and eating saltine crackers= failure. Set a SHORT term future goals but don’t be consumed with it. Move on to your next goal WHEN you finish the previous one. Time gaps=frustration=failure. Do what you can NOW not tomorrow. Writing things down will help lessen the mental time gaps.
  • Modify the way you are doing things if you are losing more than 3lbs a week. More than that indicates water loss and that is your worst enemy when trying to lose fat weight.
  • Know that you may feel really bad during the initial phases. Toxins and hormones are stored in fat so when fat breaks down, the toxins/hormones will be released into the blood therefore effecting the rest of the body. Because of this, lab tests should be done to see how well your liver and kidneys can detoxify those toxins and hormones. Additional supplementation may be needed.
  • Supplements can be of extreme value when trying to lose weight. What type and dosages can be determined by simple blood and/or urine test.
  • Having a doctor to optimize weight loss and guide you is recommended. Too often people end up quitting and saying ” it didn’t work for me”. Don’t let that be you.

  • Ritalin is generally used to increase concentration and slow motor movement.
  • Ritalin works by increasing dopamine levels in the brain. It has the same mechanism as cocaine.

I’m sure you have heard it a few time, “I have X, Y, Z condition…It’s genetic!” The fact is that only a few conditions such as Huntington’s Disease, Tay-Sachs Disease, and Down Syndrome are considered genetic disorders.  “What about the rest? I just know that my diabetes, cholesterol issues, sleeping problems, high blood pressure, and the tumor I had removed last week are genetic…my mom had it!”

It’s not that you have a certain gene but it’s the variation of that gene that makes it easier for you to develop a dysfunction that leads to disease. These genetic variations are called Single Nucleotide Polymorphisms or SNP’s. In essence, they are what make you biologically different from your neighbor. When your cells divide they must copy their DNA. They should look exactly the same, however, variations commonly occur that ultimately determines how humans develop diseases and respond to pathogens (things that cause disease), chemicals, drugs, vaccines, and other agents.

Here is an example of a SNP:  Let’s say that a particular gene’s purpose was to process Folic Acid. If you have a genetic variation that causes the way you process Folic Acid, you may need to supplement 500x more Folic Acid just to function correctly. Your run-of-the-mill multi-vitamin or prenatal vitamin won’t cut it. Folic Acid is needed for proper cell division/repair which is a huge problem in cancer patients and women planing to become pregnant. Those with SNP’s that disturb Folic Acid  may also be prone to anemia, cause neurological problems in developing fetuses, and disrupt behavior such as ADD, anxiety disorder, and depression.  Thanks to the Human Genome Project we were able to identify thousands of SNP’s that play a role in a person’s chances to develop diseases.

SNP’s can effect:
•    Hormonal/Endocrine System- Estrogen Positive Cancers, Endometriosis, Pre/Post Menopause  Complications, Osteoporosis, Cardiovascular Disease, Inflammation
•    Cardiac system- Cardiovascular Diseases, Cholesterol, Blood Pressure, Oxidative Stress
•    Detoxification System- Cancers, Chronic Fatigue, Adverse Drug Reactions, Allergies
•    Energy Production System- Cancers, Chronic Fatigue, Fibromyalgia,
•    Neurological System- Alzheimer’s, Parkinson’s Disease, Depression, Anxiety, Insomnia, Seizures, ADD, Autism

Hashimoto’s Thyroiditis and the Immune System- The Functional Medicine Approach April 8, 2008

Summary

  • Thyroid Gland- Gland that sits just below your “Adams Apple”. It helps control rate of energy production and conservation.
  • Hypothyroidism- “Turning down the thermostat” for most of your biological activities.
  • Hashimoto’s Thyroiditis is an auto-immune disease where your body attacks it’s own Thyroid tissue. Imbalances are seen in the immune system’s TH1 / TH2 responses, imbalances that typically come about when chronically exposed to an antigen (virus, bacteria, foods, everyday chemicals ect.) Immune Imbalances can lead to auto-immune diseases like Hashimoto’s.

RECOMMENDED BLOOD TESTS FOR HASHIMOTO’S (INCLUDING TPO ANTIBODIES)

  • Suspect Hashimoto’s if you are on medications for hypothyroidism like Synthroid and they “just don’t work”.
  • Functional Medicine Practitioners may test for Thyroid Peroxidase Antibodies (TPO Antibodies) and Thyroglobulin Antibodies (TGA Antibodies), which are the white blood type cells that attack the Thyroid gland.
  • Most of people with Hashimoto’s have an inborn error in processing vitamin D, which is important in the stability of the immune system. One should conceder ordering a vitamin D test (25 Hydroxy D) to check for optimal vitamin D levels (50-70 ng/ml).
  • Situations that make Hashimoto’s worse:
    • Gluten Intolerance- 95% of symptoms will go away when gluten is eliminated!
    • Iodine- This can increase oxidative stress on the Thyroid (see Jodbasedow phenomenon).
    • Sugar handling issues (hypoglycemia/insulin resistance/diabetes).
    • Intestinal infections- Theses go undetected most of the time (AKA YOU CAN NOT FEEL THEM). Stool testing is crucial.
    • Unbalanced hormones.
    • Heavy metal burdens- We all have a degree of heavy metal burden, however, higher levels of metals like mercury, arsenic, and antimony can promote major immune system dysfunction. You yourself can test for heavy metal burden with a simple hair test.
  • Goals:
  1. To stabilize antibody spikes by balancing the immune system. Understand your imbalance by ordering two blood tests:
    1. TH1/TH2 Balance Panel
    2. CD4/CD8 Ratio Panel
  2. Reduce oxidative damage done by the auto-antibodies

  • Vitamin A and D are needed to allow thyroid hormones to work.
  • Vitamin A has to be in the form of retinoic acid (still vitamin A). This is done by enzymes in the intestines.
  • Intestinal dysfunctions is the most overlooked condition in America.
  • As much at 70% (90% in my practice) are severely deficient in vitamin D when levels are checked in the blood.
  • What to we do? A few things:
    • How is your gut functioning? Specific lab tests can be done to find out. Find and build a relationship with a doctor that practices Functional Medicine. Most doctors are only concerned with disease therefore function will be overlooked.
    • Make it a point to go outside for AT LEAST 15 minutes a day WITHOUT sunscreen. Longer than 30 minutes? Combination of coconut oil and iodine (careful of iodine Hashimoto Hypothyroidism people). Commercial sunscreens are toxic and it’s compounds can easily be absorbed through the skin. Many of these compounds can alter thyroid function. Do your homework when using them.
    • Supplement vitamin D: At least 2500 IU/day, upwards of 4,000 IU/day for severe deficiencies (several months). Get your vitamin D levels checked to find out if you’re deficient. Buy vitamin D (all products) from a professional sources (doctors). Those companies are held to stricter standards. DO NOT PURCHASE FROM DISCOUNT STORES ECT.
    • Since these suggestions are very simplistic, getting thoroughly checked out by a Functional Medicine practitioner will expose the web of factors that is driving your hypothyroidism.

Prostate Cancer. The Functional Medicine Approach March 21, 2008

Summery:

  • Men produce estrogen too! Estrogen can not be eliminated from the body as itself so it is broken down in the liver into 2 4 and 16 hydroxyestradiol (2, 4, and 16 for short) so it can be.
  • 2 are “good” for the prostate and 4, 16 are “bad”.
  • Prostate tissue is negatively effected by the 4 hydroxyestradiol and can cause the glands cells to divide rapidly–>cancer
  • Compounds like I3C, 3,3′-Diindolylmethane (AKA: DIM for those who can’t digest I3C properly) help the liver produce a lot more 2’s thus decreasing risk of prostate cancer…by restoring balanced function.
  • Insulin resistance (“pre-diabetes”) causes men to convert their testosterone into estrogen making prostate cancer risk increase dramatically and further cause worst insulin resistance. Insulin support, diet changes, and compounds such as Chrycin (slows testosterone–>estrogen) can be use to address this nasty vicious cycle.
  • Immune system breakdown is seen in virtually ALL cancer patients. Immune support is a must. Although a full program must be in place by a Functional Medicine physician like Dr. Brady Hurst, compounds such as MGM-3 has been shown to stimulate Natural Killer Cells, immune cells that kill cancer cells.

Today, as over 110,000 people a year are dying from adverse drug interactions (now the 4th leading cause of death in America, right behind stroke deaths), more and more people are turning away from the traditional medical care model and gravitating toward alternative healthcare physicians such as specialized Chiropractors and Doctors of Naturopathy to take care of them and their families. This includes specialized laboratory findings, in depth medical and personal histories, and detail exams in order to see how the patient is able to adapt to his/her environment not just to determine if the patient has a disease.

One example is assessing the risk for breast cancer. Rachel and her daughter (18 years of age) came to my office following one of my lectures on breast cancer. Many of their family members either had or currently have breast cancer. After getting detailed information about her lifestyle, I decided to run an Estronex™ test. This simple urine test measures to see if her body is breaking down the hormone estrogen correctly. We know from the professional literature that waste products of estrogen breakdown (4-hydroxyestrone and 16-x-hydroxyestrone) are very toxic to breast and cervical tissues and can play a major role in the development of cancer. The good news is that another breakdown product of the same estrogen (2-hydroxestrone) had anti-cancer properties. Rachel’s daughter was not pumping out much of this “good” estrogen but did have large amounts of this “bad” estrogen. Because of her dreadful lifestyle and her “genetic individuality”, her body was not able to make enough of these good estrogens. We made changes in her lifestyle habits and used strong supplements that have been shown in the literature to promote “good” estrogen production. Then, four months later we re-tested her and not only did she feel like an entirely new person, we were able to see through the laboratory test that she was producing larger amounts of “good” estrogen. True, she did not have cancer at the time I first saw her, however, I knew that her risks of developing cancer within 20, 30, 40 years were very high. We focused on restoring health to her broken system not just determining if she had cancer or not (current healthcare model). Rachel’s family now knows the importance of having a doctor of Functional Medicine monitor their health.

Many MD’s are great at treating people in emergency situations and using drugs to relieve short term symptoms, however, fails to adequately determine if the person’s ability to heal itself is quality or not.

I say this because all doctors take the Hippocratic Oath, “If you can not do good, at least do no harm.” This oath is being extremely ignored by many of today’s healthcare professionals because of being only symptom focused.
